Get a foot and half long strong wire and place a little treat on top...it works

remove any other surfaces that he might confuse for peeing ground, like carpets, any fabric, paper, newspapers, flower pots, curtains etc. Also, put the box in a corner or somewhere where he has some privacy. IMPORTANT: change the sand VERY often!! Cats will not go and stand on the sand if it's wet or smelly!!

Some cats don't like to poop and pee in the same box - or maybe they don't like the cat litter you are using. You said your cat is an adult - has he done this all his life?
