    • On a simple level: you see a dog with a quality you want, you breed that dog. You see a dog that lacks the qualities you want, you don't breed that dog. With a end goal in mind, eventually you refine down to what you're looking for. However, in reality it's much more complicated than just that. You have to know exactly what you want, first. You also have to know the dogs you are using and you have to have a really, really good grasp of genetics. You must know the genetic lineage of a dog down to the levels of what each dog possesses that is good as well a what each dog possesses that is bad and you have to make a lot of judgement calls. Inbreeding doesn't occur, rather linebreeding does often take the place. Linebreeding is breeding dogs that are somewhat distantly related with a clear goal in mind. Inbreeding is allowing dogs to mate with their parents, siblings, etc. out of idiocy and all sorts of things go wrong.

    • It greatly depends. All dog breeds were inbred when first starting so technically all dogs of a specific breed can trace their ancestry back to the fore founders(one male and one female). Some breeds, such as Yorkshire Terriers and Doberman Pinschers were created as recently as 150 years ago from several breeds mixing together continuously and forming one specific breed, now their are breed standards for both of these dogs. This shows you how quickly a dog breed can be created. Also many dog breeds are named after the people who created them so this shows that in a life time one human or a group of humans can form a breed. I hope this answers your question!
